29 CFR 1926.250(b)(1): Material inside buildings under construction was stored within 6 feet of hoist ways or inside floor opening:     a) Throughout of the Mezzanine Level - Materials, such as but not limited to sheets of plywood, tools and scrap debris, used by the carpenters were stored on the mezzanine level directly at the edge of the floor exposing workers working below to a struck by hazard, on or about 03/13/13.     b) Carpenters were observed walking next the edge of the mezzanines floor where materials were stored with one foot from the floors edge exposing them to struck-by hazards, on or about 03/13/13.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems     a) Mezzanine Level - Carpenters were observed without fall protection while installing built up floor within two of the edge of the deck without using fall protection exposing them an 8 foot 10 inch fall hazard, on or about 03/13/13.

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:     a) Northeast and Northwest Corners of the Mezzanine Level - Carpenters were observed without fall protection installing built up floor within five feet of the unprotected stairwell/hole without using fall protection exposing them an 8 foot 10 inch fall hazard into the stairway opening, on or about 03/13/13.

29 CFR 1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with a stair rail system along each unprotected side or edge:     a) Between the Mezzanine Level and the Ground Level - A foreman and carpenters were observed using the existing stairway which was not equipped with a handrail on the open side of the stairs exposing them to an 8 foot 10 inch fall hazard, on or about 03/13/13.
