29 CFR 1910.147(c)(4)(i): Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Okamoto Sandusky Manufacturing, LLC, located at 3130 West Monroe St., Sandusky, OH 44870: On or about March 25, 2013, the employer failed to utilize machine-specific energy control procedures to control hazardous energy of the Foam Oven Line (manufactured by Kwan Chin Machine Industrial Co., Ltd.) located in the operations area. Employees did not control all hazardous energy sources such as, but not limited to, electrical, water, and pneumatic sources, while performing activities such as, but not limited to, changing embossing rolls. Employees were exposed to caught-in and struck-by injuries. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: a) Okamoto Sandusky Manufacturing, LLC, located at 3130 West Monroe St., Sandusky, OH 44870: On or about March 25, 2013, the employer failed to implement established energy control application steps to control hazardous energy of the Foam Oven Line (manufactured by Kwan Chin Machine Industrial Co., Ltd.) during a time employees were changing embossing rolls. Employees were exposed to caught-in and struck-by injuries because all hazardous energy sources such as, but not limited to, electrical, water, and pneumatic source were not controlled prior to conducting activities such as, but not limited to, changing embossing rolls. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: a) Okamoto Sandusky Manufacturing, LLC, located at 3130 West Monroe St., Sandusky, OH 44870: On or about March 25, 2013, the employer did not ensure that periodic inspections of the energy control procedures were conducted at least annually to ensure that they were properly implemented during the lockout/tagout of the equipment such as, but not limited to, the Foam Oven Line (manufactured by Kwan Chin Machine Industrial Co., Ltd.). Employees were exposed to caught-in and struck-by injuries in a case of the lockout/tagout procedures not being properly implemented. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.
