29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Okamoto Sandusky Manufacturing LLC, on or about March 5, 2024, at the address of 3130 W. Monroe St. Sandusky, OH; The employer did not ensure the Lam 5 coater rollers were guarded in such a manner as to prevent employees' hands from reaching the ingoing nip points created by the adhesive pinch rolls. b) Okamoto Sandusky Manufacturing LLC, on or about March 5, 2024, at the address of 3130 W. Monroe St. Sandusky, OH; The employer did not ensure the Lam 5 winder was guarded in such a manner as to prevent employees' hands from reaching the ingoing nip points created between the Winder's rotating shaft and product. Okamoto Sandusky Manufacturing LLC was previously cited for a violation of Occupational Safety and Health standard 29CFR 1910.212(a)(1) which was contained in OSHA inspection #1494164, citation 1, item number 1, and was affirmed as final order on February 18, 2021, with respect to a workplace located at 3130 W. Monroe St. Sandusky, OH 44870.
