Okamoto Sandusky Manufacturing, LLC located in Sandusky, Ohio:  On or about November 12, 2014, the employer did not ensure the Mitsubishi forklift #27, model # 2FBC25, was removed from service when it discovered the head lights were not working.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in the activities covered by this section.     a. Okamoto Sandusky Manufacturing, LLC located in Sandusky, Ohio:  On or about November 12, 2014, the employer did not ensure calender operators were utilizing lockout procedures to isolate the energy sources on the embossing roller and the cold roller when exposed to crushing hazards while sanding the #4 calender roller with an abrasive pad.      b. Okamoto Sandusky Manufacturing, LLC located in Sandusky, Ohio:  On or about November 12, 2014, the employer did not ensure calender operators were utilizing lockout procedures to isolate the energy sources on the cold roller when exposed to crushing hazards while wiping down the rubber roller during production.    Okamoto Sandusky Manufacturing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(4)(i),  which was contained in OSHA inspection number 897496, citation number 1, item number 1a and was affirmed as a final order on August 22, 2013, with respect to a workplace located at 3130 West Monroe St., Sandusky, Ohio.

An orderly shutdown was not utilized to avoid any additional or increased hazard(s) to employees as a result of the equipment stoppage.    a. Okamoto Sandusky Manufacturing, LLC located in Sandusky, Ohio:  On or about November 12, 2014, the employer did not ensure employees were trained on the purpose and function of the energy control program.  Employees were exposed to crushing hazards from the cold roller and the embossing roller while sanding the #4 calender roller and wiping down the rubber roller.    Okamoto Sandusky Manufacturing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(7)(I),  which was contained in OSHA inspection number 897496, citation number 1, item number 3 and was affirmed as a final order on August 22, 2013, with respect to a workplace located at 3130 West Monroe St., Sandusky, Ohio.
