29 CFR 1926.501(b)(1): Each employee on a walking/working surface having an unprotected side or edge which was six feet (1.8m) or more above a lower level was not protected from falling by the use of a guardrail system, a safety net system, or personal fall arrest system. (a)A&M Steel, Inc. @ 5440 Arapahoe, Boulder, CO 80303: On and before March 26th, 2013, the exposing employer did not ensure each employee on a walking/working surface having an unprotected side or edge which was six feet (1.8m) or more above a lower level was protected from falling by the use of a guardrail system, a safety net system, or personal fall arrest system in that, an employee was engaged in welding operations on the edge of a roof without fall protection. This condition exposed the employee to a fall hazard of 8'3".

29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purposes for which they were designed: (a) A&M Steel, Inc. @ 5440 Arapahoe, Boulder, CO 80303: On and before March 26th, 2011, the employer allowed the portable step-ladder to be leaned against a column to allow an employee to weld which is contrary to the manufacturer's instructions. The ladder was designed for the spreaders to be opened and locked during use. This hazard exposed the employee to falls from heights of approximately 8 foot 3 inches.
