29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: (a) Roof over garage, new residential construction located at 4227 Hardy Avenue, Smyrna, GA 30080: Employee was exposed to fall hazards while installing DOW Weathermate Housewrap on a 6/12 pitched hip style roof with eave heights of approximately 10' with no fall protection system in place.

29 CFR 1926.1053(b)(10): The tops of a non-self-supporting ladder were not placed with the two rails supported equally: (a) West side, new residential construction located at 4227 Hardy Avenue, Smyrna, GA 30080: Employees used a 32' Werner (Model #D1532-2) Aluminum Extension Ladder with the top right side rail not supported against the structure to prevent accidental movement and collapse of the ladder. Employee working on the ladder at heights of approximately 17' installing house wrap was exposed to fall hazards to the ground below.
