29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact:    On or about April 1, 2013, at the above addressed jobsite, employees were applying asphalt onto a residential roof and were not wearing protective gloves exposing them to burn hazards.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:    On or about April 1, 2013, at the above addressed jobsite, employees were applying asphalt onto a roof and were not protected from fall hazards of approximately 27 feet.    Tim Graboski was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 311090443, citation number 01, item number 001, issued on 8/6/2008, with respect to a workplace located at 4570 S. Military Trail, West Palm Beach, Florida 33463, which became a final order on 9/19/2008.
