29 CFR 1910.36(d)(1): Employee(s) were not able to open an exit route door from the inside at all times without keys, tools, or special knowledge:    a)      Hotel, 32 Wing on 2nd floor, west exit door:  On or about April 3, 2013, the single exit door was locked with a dead bolt requiring a special knowledge to locate the bolt and slide it out or pull it in the proper direction to open the door.        b)      Hotel, 42 Wing on 3nd floor, west exit door:  On or about April 3, 2013, the single exit door was locked with a dead bolt requiring a special knowledge to locate the bolt and slide it out or pull it in the proper direction to open the door.

29 CFR 1910.157(e)(2): Portable fire extinguishers were not visually inspected at least monthly:(a)    Hotel 31 Wing, Linen Room:  On April 3, a fire extinguisher was not inspected on a monthly basis. There was no tag showing a monthly or visual checked. None of the fire extinguishers in the hotel received a monthly inspection checked. A fire extinguisher found in the main lobby, the gauge was off the green mark and needed recharging.

The employer did not record the annual maintenance date and retain this record for one year after the last entry or the life of the shell, whichever is less.     a)         Hotel 31 Wing, Linen Room:  On or about April 3, a fire extinguisher was not inspected annually. None of the fire extinguishers in the hotel received an annual inspection. Another fire extinguisher found in the main lobby, the gauge was off the green mark and needs recharging.

A sign on the wall left of the panel said, Keep this area clean.    b. High Voltage Room, 33Wing. This violation occurrwd on April 3, 2013 when carpet stored on floor were blocking access to electrical control circuit panel.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:    Hotel roof top:  This violation was observed on April 3, 2013 when a flexible (black) extension cord was laying on the roof top and run the length of the flat roof providing power to an outdoor emergency light. The black extension cord was exposed to the weather element and used as substitute for fixed wiring.
