29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) Morehead Ave. Conshohocken, Pa, Lot 4, Roof: On or about March 20, 2013,, the rails of a portable extension ladder, used by employees for access to a roof, did not extend 3 feet above the upper landing surface and was not secured at its top to a rigid support, exposing employees to a fall hazard of no less than 25 feet.
