29 CFR 1926.20(b)(2): The employer did not develop an accident prevention program, that provided for frequent and regular inspections of the job sites, materials, and equipment, to be made by competent persons designated by the employer.       a)  3 Buttonwood Court, Moorestown, NJ:  The employer did not initiate a program for a competent person to conduct frequent and regular inspections of the jobsite, to identify hazards and/or stop the work in the event workers were placed in a hazardous condition. As such, B&S Carpentry & Roofing employees were conducting roofing activities on the roof of a multi-story new residence under construction without any form of fall protection.  The side rails of the portable ladders used to access the roof were not properly extended 3 feet above the landing surface.  Employees were exposed to a fall hazard of greater than 24 feet from the roof's eave to the ground.  Observed on or about 10/22/2013.       NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who was exposed to fall hazards that enabled each employee to recognize the hazards of falling and train each employee in the procedures to be followed in order to minimize these hazards.     a)  3 Buttonwood Court, Moorestown, NJ:  B&S Carpentry & Roofing employees were conducting roofing activities on the roof of a multi-story new residence under construction without fall protection and were exposed to a fall hazard of greater than 24 feet from the roof's eave to the ground.  The employer had not trained employees exposed to fall hazards to recognize those hazards and the procedures to be followed to minimize the hazards.  Observed on or about 10/22/2013. 29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.     a)  3 Buttonwood Court, Moorestown, NJ:  B & S Carpentry & Roofing employees were conducting roofing activities on the steep sloped roof of a multi-story new residence under construction without any form of fall protection.  Employees were exposed to a fall hazard of greater than 24 feet from the roof's eave to the ground.  Observed on or about 10/22/2013.        The employer, B & S Carpentry & Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 898926, citation number 01, item number 001, and was affirmed as a final order on 06/12/2013, with respect to a workplace located at 149 Morehead Ave, West Conshohocken, PA.    The employer, B & S Carpentry & Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(11), which was contained in OSHA inspection number 113740492, citation number 01, item number 001, and was affirmed as a final order on 09/21/2011, with respect to a workplace located at 676 East Main St., New Holland, PA.    NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
