29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) 211-217 Heathersage Road, Marietta, PA - On or about August 13, 2024, employees replacing a residential 8/12 pitch roof were not utilizing acceptable means or methods of fall protection, exposing employees to a fall hazard of approximately 23 feet to grade.  b) 211-217 Heathersage Road, Marietta, PA - On or about August 13, 2024, employees replacing a residential detached garage roof were not utilizing acceptable means or methods of fall protection, exposing employees to a fall hazard of approximately 9 feet to grade.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) 211-217 Heathersage Road, Marietta, PA - On or about August 13, 2024, employees replaced the roof of an 8/12 pitch residential structure at a height of approximately 23 feet and were not trained in appropriate fall protection methods to avoid hazards associated with elevated work areas.  b) 211-217 Heathersage Road, Marietta, PA - On or about August 13, 2024, employees replaced the roof of a residential detached-garage at a height of approximately 9 feet and were not trained in appropriate fall protection methods to avoid hazards associated with elevated work areas.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) 211-217 Heathersage Road, Marietta, PA - On or about August 13, 2024, employees were exposed to a fall hazard of approximately 23 feet due to the ladder on the side of the house not being extended 3 feet above the landing.  b) 211-217 Heathersage Road, Marietta, PA - On or about August 13, 2024, employees were exposed to a fall hazard of approximately 23 feet due to the ladder on the rear of the house not being extended 3 feet above the landing.  c) 211-217 Heathersage Road, Marietta, PA - On or about August 13, 2024, employees were exposed to a fall hazard of approximately 9 feet due to the ladder on the detached garage not being extended 3 feet above the landing.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
