29 CFR 1926.1053(b)(1): Because of the portable ladders length, the ladder side rails did not extend to at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigid support, and a grasping device, such as a grabrail, was not provided to assist the employees in the mounting and dismounting the ladder:      (a)  Urbina Construction, Inc. at 1946 Walnut St., Boulder, CO 80302:  On 4/1/13, the exposing employer did not ensure that employee was utilizing portable ladders in such a way that enabled them to access and egress from the roof using ladders that extended 3 above the walking and working surface. Employees were using a portable extension-ladder for access to remove old decking. Employees were exposed to a fall of 10 feet while attempting to use the ladder.     Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8m) or more above lower levels were not protected by guardrail systems, safety net system, personal fall arrest system, or other approved alternative fall protection system:      (a)  Urbina Construction, Inc. at 1946 Walnut St., Boulder, CO 80302:  On 4/1/13, the exposing employer did not ensure that employees were protected from injury, in that employees were removing old decks in preparation for installation of new decks on a multi-family home were not utilizing a fall protection system. This condition exposed employees to a fall hazard of approximately 14 feet.      Abatement Note:  Abatement certification and documentation are required for this item (see enclosed Certification of Corrective Action Worksheet)    Urbina Construction, Inc. was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection #98492, Citation 1, Item 1, issued on 9/28/11, and became final order on  10/06/11 with respect to a workplace located at 2366 Park Lane, Louisville, CO 80027.
