29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels were not protected by guardrail systems, safety net system, personal fall arrest system, or other approved alternative fall protection system:      (a) Urbina Construction, Inc. at 1955 W 34 Ave, Denver, CO 80211:  On or before March 31, 2015 the exposing employer did not ensure that employees were protected from injury, in that three employees setting joists on the third level worked near the edge of the building without utilizing a fall protection system.  This condition exposed the employees to a fall hazard of approximately 23 feet.      (b) Urbina Construction, Inc. at 1955 W 34 Ave, Denver, CO 80211:  On or before March 31, 2015 the exposing employer did not ensure that employees were protected from injury, in that an employee walked on an I-beam support on the third level without utilizing a fall protection system.  This condition exposed the employee to a fall hazard of approximately 10 feet.      (c) Urbina Construction, Inc. at 1955 W 34 Ave, Denver, CO 80211:  On or before March 31, 2015 the exposing employer did not ensure that employees were protected from injury, in that an employee worked near the top of an exterior wall on the third level without the use of a fall protection system.  This condition exposed the employee to a fall hazard of approximately 30 feet.       Urbina Construction, Inc. was previously cited for a violation of the same or substantially similar condition of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13) which was contained in OSHA Inspection Number 899082, Citation Number 2 Item 1 issued on April 15, 2013, and became Final Order on May 21, 2013 with respect to a workplace located at 1946 Walnut St., Boulder, CO 80302.
