29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used: a. On or about April 5, 2013, at the above addressed jobsite, employees were exposed to falls of approximately 18 feet while accessing/egressing the ladder jack scaffold platform using the support ladders which required them to crawl over the platform from the ladder.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure: a. On or about April 5, 2013, at the above addressed jobsite, employees working from the 6/12 pitched roof on the east side of the building were exposed to falls of approximatley 19.7 feet due to the lack of a fall protection system. E. J. Construction was previously cited for a violation of this Occupational Safety and Health standard, 1926.501(b)(13), which was contained in OSHA inspection number 313238412, citation number 1, item number 2 and was affirmed as a final order on July 7, 2009, with respect to a workplace located at 1519 Walnut in Arlington Heights, IL.
