29 CFR 1926.150(c)(1)(vi): A fire extinguisher, rated not less than 10B, was not provided within 50 feet of wherever more than 5 gallons of flammable or combustible liquids or 5 pounds of flammable gas are being used on the jobsite:     (a)Crystal Pond Rd at Madison Place Southborough, MA: On or about April 10, 2013, there was no fire extinguisher present to meet the requirements of this subpart when more than 5 gallons of flammable/combustible liquids, in this case 5 + gallons of gasoline to power air compressors, are being used on a job site:      Mota Construction Corp was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.150(c)(1)(vi), which was contained in OSHA inspection number 838343, citation number 1, item number 1, and was affirmed as a final order on March 13, 2013, with respect to a workplace located at 2 Adams Farm Rd Shrewsbury, MA.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system:    (a)Crystal Pond Rd at Madison Place Southborough, MA: On or about April 10, 2013, employees were observed working at two seperate buildings on this residential construction project more than 12 FT above the ground below without any fall protection as required by this subpart:    Mota Construction Corp was previously cited for a repeat violation of this occupational safety and health standard 29 CFR 1926.501(b)(13) , which was contained in OSHA inspection number 838343, citation number 2, item number 1, and was affirmed as a final order on March 13, 2013 with respect to a workplace located at 2 Adams Farm Rd Shrewsbury, MA:    Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
