(a) Location - 78 Oxbow Road, Framingham, Massachusetts: On or about January 10, 2017 an employee operating an All Terrain Powered Industrial truck was not wearing a seatbelt that the manufacturer provided with the equipment.

29 CFR 1926.602(d): The employer did not ensure that each employee operating a powered industrial vehicle had been trained and evaluated as required by paragraph 29 CFR 1910.178(l): (a) Location - 78 Oxbow Road, Framingham, Massachusetts: On or about January 10, 2017 the employer did not ensure that an employee who was operating an All Terrain Powered Industrial truck was trained to operate the equipment. Mota Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.602(d), which was contained in OSHA inspection number 903955, citation number 01, item number 005 and was affirmed as a final order on 12/31/2015, with respect to a workplace located at 10 & 12 Adams Farms Road.
