29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:     a) Housekeeping/Laundry - Noted on  4/12/13 and at times prior thereto, it was noted that the employer did not develop a written Hazard Communication Program for employees who were exposed to the hazards of housekeeping products like Fabuloso, bleach, Sun Detergent, Zep Acidic toilet bowel cleaner, and Zep degreaser.

29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification which included the requirements as outlined in 29 CFR 1910.132(d)(2):  a) Housekeeping/Laundry - Noted on or about 4/12/13 and at times prior thereto, the employer did not prepare a workplace hazard assessment for employees using cleaning and laundry products requiring the use of personal protective equipment (PPE).  The following elements shall be addressed in the workplace hazard assessment:        1.  Select and have each affected employee use the types of PPE that will protect them from the hazards identified in the hazard assessment;    2.  Communicate the selection decisions to each affected employee and;  3.  Select PPE that properly fits each affected employee.          *ABATEMENT NOTE:  The employer shall verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment.

29 CFR 1910.305(e)(1): Cabinets, cutout boxes, fittings, boxes, and panelboard enclosures in damp or wet locations were not installed to prevent moisture or water from entering and accumulating within the enclosures:  a)  Pump Room- Noted on or about 4/12/13 and at times prior thereto, the circuit breaker panel and disconnects were not effectively closed.
