29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, and service rooms shall be kept clean and orderly and in a sanitary condition. a. At the workplace in the Gen-Fab Department, employees were exposed to tripping hazards by pallets stored in passageways. Employees had to walk on top of pallets to place parts on storage racks.

29 CFR 1910.179(n)(2)(ii): The load shall be attached to the load block hook by means of slings or other approved devices. a. In the Gen-Fab Department at the workplace, employees were exposed to struck-by hazards from parts secured to the load block by devices that were not rated for lifting or hoisting. Employees used c-clamp style vices to lift parts weighing 500 to 1000 lbs with an overhead crane.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. a. At the workplace in the Gen-Fab Department, employees were exposed to flying metal debris without the protection of point of operation guarding. Metal parts that have been banded or fused by heat have come apart while being machined on lathes.
