29 CFR 1910.132(d)(1): The employer shall assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE). If such hazards are present, or likely to be present, the employer shall: 29 CFR 1910.132(d)(1)(i): Select, and have each affected employee use, the types of PPE that will protect the affected employee from the hazards identified in the hazard assessment: a. On or about May 18, 2015, the employer did not select and have each affected employee use the proper personal protective equipment (PPE) to protect against a thermal contact and burn hazard at the 3-High Rolling Mill and furnace #1 such as, but not limited to, the following: face shield, hard hat, apron, adequate sleeves or long sleeve heat resistant/flame retardant shirt, heat resistant/flame retardant pants, or other protective combination. b. On or about May 18, 2015, the employer did not select and have each affected employee use the proper PPE to protect against a thermal contact and burn hazard at the 2-Stand Rolling Mill and furnace #2 such as, but not limited to, the following: apron, adequate sleeves or long sleeve heat resistant/flame retardant shirt, heat resistant/flame retardant pants, or other protective combination.
