29 CFR 1910.138(a): The employer did not select and require employee(s) to use appropriate hand protection when employees' hands were exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasion; punctures; chemical burns; thermal burns; and harmful temperature extremes:  a)  Wave Solder Machine:  The operator stuck his hand and arm into the machine for cleaning and was not required to wear appropriate personal protective equipment (e.g., elbow length thermal resistant gloves or equivalent); to protect against thermal burns on or about 4/10/13.         b) Wave Solder Machine:  The operator handled a magnifying glass attached to a metal base which was contaminated with lead and was not required to wear appropriate personal protective equipment (e.g., chemical resistant gloves or equivalent) to protect against lead exposure; onor about 4/10/13.

29 CFR 1910.1025(h)(1): All surfaces were not maintained as free as practicable of accumulations of lead:  a) Wave Solder Area:  N95 dust masks were stored loosely on top of the flammable storage cabinet which contained 222.2 micrograms (ug) of lead per 100 square centimeters (cm2) surface area and was not maintained as free as practicable of accumulations of lead; on or about 3/26/13.  b) Wave Solder Area:  The floor next to the wave solder machine contained 542.6 micrograms (ug) of lead per 100 square centimeters (cm2) surface area and was not maintained as free as practicable of accumulations of lead; on or about 3/26/13.  c) Wave Solder Area:  The metal base of the magnifying glass used to examine circuit boards that had just been soldered contained 5,488 micrograms (ug) of lead per 100 square centimeters (cm2) surface area and was not maintained as free as practicable of accumulations of lead; on or about 3/26/13.  d) Electronic Assembly Area Outside of Wave Solder Room:  An employee ate lunch at a desk that contained 154.9 micrograms ug) of lead per 100 square centimeters (cm2) surface area and the desk was not maintained as free as practicable of accumulations of lead; on or about 4/10/13.  e) Electronic Assembly Area Outside of Wave Solder Room - Lead Soldering Table:  An employee stored a wrapped granola bar in a desk drawer below the table that contained 5,998 micrograms of lead per 100 square centimeters (cm2) surface area and was not maintained as free as practicable of accumulations of lead; on or about 4/10/13.
