29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces shall be protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. a. On or about April 16, 2013 - at the above addressed jobsite, employees conducting roofing operations on a low slope roof, approximately fourteen (14) feet six (6) inches above the ground, were exposed to falls to the inside of the building created by the presence of unprotected skylights.

29 CFR 1926.502(h)(1)(v): The safety monitor shall not have other responsibilities which could take the monitor's attention from the monitoring function. a. On or about April 16, 2013 - at the above addressed jobsite, an employee was exposed to a fall hazard of approximately fourteen (14) feet six (6) inches to the ground due to the safety monitor having other responsibilities, such as performing roofing operations with other employees. W.B.R. ROOFING COMPANY, INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 1926.501(b)(10), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 312594252, CITATION NUMBER 1, ITEM NUMBER 2 AND WAS AFFIRMED AS A FINAL ORDER ON JULY 6, 2009 WITH RESPECT TO A WORKPLACE LOCATED AT 222 WEST COURT ST., KANKAKEE, IL 60901.
