The path to ground from circuits, equipment, and enclosures shall be permanent and continuous.   a)     On or about April 18, 2013; at the center of the jobsite, the employer allowed employees to utilize a rotary hammer (hand-held hammer drill) with a plug that was a missing ground pin.  Employees used the rotary hammer to drill a hole in the wall in order to install electrical wiring.  The missing ground pin prevented the rotary hammer from having a continuous path to ground.
