29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: At 153 Unit B, East Harmon Industrial Park on April 17, 2013, eight foot front-end attachments were used with the Hyster Forklift, Serial Number D004V10287S without written approval from the manufacturer of the powered industrial truck. Operator utilized the 8 fork attachment in order to lift a PVC pipe, approximately 21 feet long.
