At the front side of the Magellan Dining Facility, Building 25010 on November 13, 2013, a midrail was not installed between the top edge of the guardrail system and the walking/working surface.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided: At the front side of the Magellan Dining Facility, Building 25010 on November 13, 2013, the distance from the ground to the first rung of the built in ladder on the frame type scaffold was approximately 24 inches. Employees used the built in ladder to access the working platform.
