29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Cross braces shall not be used as a means of access. a. On or about April 22, 2013, at the above addressed jobsite, employees conducting masonry operations from a tubular welded framed scaffold, eleven (11') feet three (3") inches above the ground, were not provided with an access ladder, thereby exposing the employees to falls.

On or about, April 22, 2013, at the above addressed jobsite, an employee conducting masonry operations from a tubular welded framed scaffold, eleven (11) feet eight (3) inches above the ground, was exposed to fall hazards due to the lack of a guardrail system at the end of the outrigger platform.

29 CFR 1926.51(f): Adequate washing facilities were not provided for employees engaged in the application of paints, coating, herbicides, or insecticides, or in other operations where contaminants might have been harmful to the employees: a. On or about April 22, 2013, at the above addressed jobsite, adequate washing facilities were not available for employees whose duties included working with mortar which contained Portland Cement. None-alkaline soap was not provided, thereby exposing the employees to chemical burns resulting from contact with Hexavalent chromium.
