29 CFR 1926.454(a):  The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:    Employees working from a frame scaffold installing durarock cement board were exposed to a fall hazard and the scaffold did not contain a mid-rail and was not fully decked. The employer had not provided training to the employees on the hazards associated with working off of the scaffolding and procedures to minimize those hazards.
