29 CFR 1926.451(c)(1): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) were not restrained from tipping by guying, tying, bracing, or equivalent means, as set forth in subparagraphs (i) - (iii) of this paragraph.    a) On October 11, 2016, employee(s) were working at a height of approximately 13.8 feet on a scaffold with a base that had a 29 inch width.  The scaffold was not restrained from tipping.

The top portion of a portable, extension ladder was used as an independent ladder to gain access to the approximately 9.6 foot high roof of the Metra station.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, shall either be immediately marked in a manner that readily identifies them as defective, or be tagged with "Do Not Use" or similar language, and shall be withdrawn from service until repaired.  (a) On October 11, 2016, an aluminum ladder was used by employees of RV Builders Corporation to access the roof approximately 9.6 feet from the ground.  Rungs of the ladder were damaged, and the ladder was not withdrawn from service.
