29 CFR 1915.506(b)(1): The employer did not physically isolate the fixed extinguishing systems or utilize other positive means to prevent the systems discharge before work was done in that space which system was located.  This violation was discovered on April 19, 2013 on board the USCG Cutter Kukui; where employees removed a 15-foot pipe with an attached valve that became dislodged and inadvertently triggered the CO2 actuater cable causing the release of approx. 600 pounds of CO2.
