29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:   Dry Dock: The yellow Hyster forklift with capacity of 6,000 lbs. was lifting personnel approximately 10 feet in the air in a jobmade manbasket that was not approved by the manufacturer.  An employee was exposed to crushed-by and fall hazards.

29 CFR  1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:  A) Mechanics' Shop: The Ballor Bench Grinder used to grind down bolts did not have a work rest attached.  Employees were exposed to struck-by hazards.  B) Pipefitters' Shop: The Setco 2HP Industrial Grinder 120-202 460V Model 460 used to grind pipes and angle irons did not have a properly adjusted work rest attached.  Employees were exposed to struck-by hazards.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  A) Mechanics' Shop: The Ballor Bench Grinder used to grind down bolts had an adjustable tongue was too far from the abrasive wheel.  Employees were exposed to struck-by hazards.  B) Pipefitters' Shop: The Setco bench grinder used to grind pipes and angle irons had an adjustable tongue guard that was too far from the abrasive wheel.  Employees were exposed to struck-by hazards.
