Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing for likely to cause death or serious physical harm to employees in that employees were exposed to:      A.) Combined Systems, Inc, Building G - Employees were potentially exposed to struck-by, projectile, and/or shrapnel/fragmentation hazards from the accidental discharge of tear gas shells.  On or about 4/23/2013, and at times prior thereto, tear gas shells were rolled down a ramp at the work table from the wet wipe side to the dry wipe side.  The process, including but not limited to, the ramp and the action of rolling the tear gas shells was not designed, installed, and/or conducted in a manner to prevent the tear gas shells primer from contacting other shells and/or objects and causing the shell to accidentally discharge/fire.
