29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee shall be protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. a. On or about April 26, 2013, at the job site located at Mill Street and Washington Street in Circleville, Ohio employees were observed working from a wooden shop made platform placed on the forks of the SkyTrak 8042 Telehandler rough terrain forklift elevated to a height of approximately 17 feet above the level below. The platform was not equipped with guardrails, and the employees were not provided personal fall protection, exposing the employees to the hazard of a fall from elevation.

29 CFR 1926.760(a)(1): Except as provided by paragraph (a)(3) of this section, each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level shall be protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems or fall restraint systems. a. At the site located at Mill St. and Washington St. in Circleville, Ohio, employees were performing metal roofing work on a commercial building with approximately no pitch and 20 feet in height near the edge with no fall protection exposing employees to a fall hazard.
