29 CFR 1926.451(c)(2)(v): Fork-lifts shall not be used to support scaffold platforms unless the entire platform is attached to the fork and the fork-lift is not moved horizontally while the platform is occupied. a) Gouge Quality Roofing, LLC located at 215 N. Court street, Circleville, OH 43113: On and preceding May 4, 2020, the controlling employer did not ensure that a forklift with an occupied, elevated platform was not moved horizontally. A JLG model 10054 rough terrain forklift was being used to lift an external platform with two siding workers inside of it while the operator drove the platform in order to reposition it. The following condition exposed the employees to fall and struck-by hazards.

A JLG model 10054 Rough terrain forklift was being used to lift an external platform at the site without a certified operator.
