29 CFR 1910.165(d)(4): All supervised employee alarm systems were not tested at least annually for reliability and adequacy:  (a) 4035 S Avenue A, Yuma, AZ 85365:  On or about 30 April 2013, and at times prior thereto, the SIMPLEX 4620 automatic supervised alarm system was not tested for reliability and adequacy since the last test conducted in June 2011, exposing the employees to hazards.

29 CFR 1904.7(b)(3): The employer incorrectly recorded the following work-related injuries or illnesses on the OSHA Form 300 or equivalent:  (a) 4035 S Avenue A, Yuma, AZ 85365:  On or about 30 April 2013, and at times prior thereto, the employer incorrectly recorded work-related injuries or illness on the OSHA Form 300 for Calendar Year 2012, in the following instances:   Instance # 1: Case number 2012-03705, on 02/22/2012, while a maintenance mechanic was  painting the lower portion of a trailer ramp in the welding shop, he felt his left knee give way with a popping sound, which resulted in  a restricted duty case. This case was incorrectly recorded both as an injury in column M1 and as an illness in column M6 , as was double counted in the total columns.    Instance #2: Case number 2012-06658, a Patrol Agent, on 06/13/2012, while participating in an Baton Instructor Certification Course, attempted to block a baton strike and experienced a pop in his left shoulder, resulting in limited mobility and restricted duty.  This was recorded as both an injury and illness in columns M1 and M6, and was counted twice in the total columns.    Instance#3: Case number 2012-07186, on 07/03/2012, the same employee identified in Instance #1, while still on restricted duty, was installing duct work atop the roof of Yuma Station, when he felt pain in the same left knee reportedly injured in Instance # 1, as he worked while kneeling, resulting in medical care.  This was recorded as both an injury and illness in columns M1 and M6, and totaled twice.    Instance#4:  Case number 2012-07421, a Patrol Agent, on 07/09/2012, while assigned as a Firearms Instructor at the Adair Firing Range, was following an agent during an ambush scenario when he tripped and fell, injuring his (unspecified) knee, resulting in restricted duty. This was recorded as both an injury and illness, and counted twice in the total columns.    Instance#5: Case number 2012-08610, a Patrol Agent, on 08/09/2012, while on a ruck march (loaded carry) along the East Main Canal, experienced pain in his left hip, which resulted in medical care. This was recorded as both an injury and illness, and counted twice in the total columns.    Instance#6: Case number 2013-00523, a Patrol Agent, on 10/02/2012, while exercising in the gym during temporary duty with the Special Operations Group in Texas, experienced a sharp pain in his left shoulder blade extending down into his left arm, resulting in medical treatment. This was recorded as both an injury and illness, and counted twice in the total columns.    Intance#7: Case number 2012-07433, a maintenance mechanic, on 07/06/2012, while relocating a mobile check point on the highway, was attacked and stung multiple times by a swarm of bees, resulting in swelling of the face, eyelids,  hands, arms, and neck  and medical treatment.  No check was made on the OSHA 300 Log to identify this event as either an injury or illness, and this case was not counted in the total column at the end of the year.    Instance#8: Case number 2013-00137, an automotive mechanic, on 09/28/2012, while filling compressed air into a tire, experienced temporary hearing loss and medical treatment when the tire exploded, but this event was neither  recorded as an acoustic trauma (injury) or  an illness (hearing loss) on the OSHA 300, and was not counted at the end of year total.  Instead the event was recorded as a skin disorder in column M2, and totaled as such and noted as a skin disorder on the OSHA 300A Annual Summary for Calendar Year 2012.
