29 CFR 1910.178(l)(4)(ii)(B): Refresher training, including an evaluation of the effectiveness of that training, was not provided to the operator when the operator has been involved in an accident or near-miss incident. a) 1105 North Park Dr., Hazelton PA; An accident occurred on 04/09/2013 whereby an employee removing pallets from a pallet stacking machine pulled over a pallet stack located next to the pallets the employee was removing with a forklift. Since the 04/09/2013 incident the employee has since been operating forklifts without having been retrained in the avoidance of any future forklift incidents, on or about 04/29/2013.
