29 CFR 1910.147(c)(6)(i): The employer did not conduct an annual or more frequent inspection of the energy control procedure to ensure that the procedure and requirements of this standard were followed:   Establishment: Lockout/tag procedures established for use in maintaining or servicing equipment pieces such as, but not limited to, CNC Komo Router, Lagun Mill, or Big Haas were not inspected in over 1 year.      Abatement certification and abatement documentation are required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citation(s).

29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by rotating edges:  Plant: Rotating edges of machines such as, but not limited to, chucks and cutting bits of mills and blades of a vertical band saw, were not guarded to prevent employee injuries.       Abatement certification and abatement documentation are required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. 29 CFR 1910.212(a)(5): Fan blade guards, where the periphery of the blades was less than seven feet above the floor or working level, had openings larger than one half inch:    Plantwide: Openings on the guards of the blades of pedestal fans with blade peripheries which were lower than 7 feet from workplace floor were approximately 2 inches wide on some fans exposing employees to amputation hazards.
