29 CFR 1910.138(a): The employer did not select and require employee(s) to use appropriate hand protection when employees' hands were exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasion; punctures; chemical burns; thermal burns; and harmful temperature extremes.    a) Bookmasters, Inc. - Ashland, Ohio: On or about April 30, 2013, the employer did not assure that employees, exposed to hot glue while operating the Horizon BQ-460 Binder wore personal protective equipment for the hands.  An employee was exposed to burn injuries when accessing the hot glue area to remove debris.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    a) Bookmasters, Inc. - Ashland, Ohio: On or about April 30, 2013, the employer did not assure that energy control procedures were developed, documented, and utilized to control hazardous energy of the Timson T32 web press.  Employees that performed press set-up, change-over and unjamming tasks were exposed to the unexpected start-up of the press while accessing hazard areas of the machine.     b) Bookmasters, Inc. - Ashland, Ohio: On or about April 30, 2013, the employer did not assure that energy control procedures were developed, documented, and utilized to control hazardous energy of the Horizon Three-Knife Trimmer (HT-70). Employees that accessed hazard areas of the machine to replace blades were exposed to amputation injury from the unexpected start-up of the trimmer.    c) Bookmasters, Inc. - Ashland, Ohio: On or about April 30, 2013, the employer did not assure that energy control procedures were developed, documented, and utilized to control hazardous energy of the Horizon BQ-460 Binder (s/n 004917).  Employees that accessed hazard areas of the machine for set-up, to refill glue, and/or to clear dried glue were exposed to hazards created by the cutting blade and/or rotating parts.

Employees that accessed hazard areas of the web press for setup and clearing jams, were not trained to lockout the machine to protect against amputation hazards.     b) Bookmasters, Inc. - Ashland, Ohio: On or about April 30, 2013, the employer did not assure that employees, who are exposed to the unexpected startup of the Horizon Three-Knife Trimmer (HT-70), were trained in the skills required for the safe application, usage, methods, and means to isolate and control energy sources. Employees were exposed to respiratory tract irritation, and eye and skin irritation when refilling ink at the Timson web press.
