29 CFR 1910.1030(g)(2)(i): The employer shall train each employee with occupational exposure in accordance with the requirements of this section. Such training must be provided at no cost to the employee and during working hours. The employer shall institute a training program and ensure employee participation in the program: A) Bookmasters, Inc., located at 30 Amberwood Parkway, Ashland, OH 44805: On or about November 13, 2017, the employer did not ensure that each employee with occupational exposure to blood-borne hazards was trained in accordance with the 29 CFR 1910.1030 standard. Such training should have taken place at the time of the employees' initial assignment, and should have covered items such as, but not limited to, a copy of the regulatory text of the 1910.1030 standard, explanations of the modes of transmission of blood-borne pathogens, and a general explanation of the epidemiology and symptoms of blood-borne diseases.

Employees did not control all hazardous energy sources such as, but not limited to, electrical before performing activities such as, but not limited to, re-webbing/webbing-up the press after the web break. Employees were exposed to amputation hazards while all hazardous energy sources were not controlled prior to conducting work. Bookmasters, Inc. was previously cited for a violation of this Occupational Safety and Health Standard (1910.147(c)(4)(i)), which was contained in OSHA Inspection # 903200, Citation number 1, Item number 2, and was affirmed as a final order on 08/16/2013, with respect to a workplace located at 30 Amberwood Parkway, Ashland, OH 44805.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): A) Bookmasters, Inc., located at 30 Amberwood Parkway, Ashland, OH 44805: On or about November 13, 2017, the employer did not effectively train employees to lockout/tagout equipment such as, but not limited to, the Timson T32 web press located in Press Department. Employees did not control all hazardous energy sources such as, but not limited to, electrical before employees conducted activities such as, but not limited to, re-webbing/webbing-up the press after the web break. Employees were exposed to amputation hazards while all hazardous energy sources were not controlled prior to conducting work. As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lock-out/tag-out device application [1910.147(d)(4)], dissipation of residual energy [1910.147(d)(5)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from amputation hazards while employees conduct activities such as, but not limited to, re-webbing/webbing-up the Timson T32 web press located in Press Department. Bookmasters, Inc. was previously cited for a violation of an equivalent Occupational Safety and Health Standard (1910.147(c)(4)(i)), which was contained in OSHA Inspection # 903200, Citation number 1, Item number 2, and was affirmed as a final order on 08/16/2013, with respect to a workplace located at 30 Amberwood Parkway, Ashland, OH 44805.
