29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems shall be made by a competent person for evidence of a situation that could result in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions. a. On or about April 30, 2013 - at the above addressed jobsite, employees conducting gas tank installation in an excavation that measured sixteen (16) feet in depth, were exposed to cave in hazards when the company failed to conduct competant person inspections of the excavation.

On or about April 30, 2013 - at the above addressed jobsite, employees performing gas storage tank installation, in an excavation that measured sixteen (16) feet in depth were not provided with protective system, thereby exposing the employees to cave in hazards.
