29 CFR 1926.95(a): Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, was not provided, used, or maintained in a sanitary and reliable condition wherever it was necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact:  a) Exterior, Southwest Corner - Respiratory protection or eye protection was not provided to masonry employees observed mixing and troweling ESSROC brick mortar which contained Portland cement, on or about 10/21/2015.  No abatement certification or documentation required for this item.

29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall or come against, was not guarded to eliminate the hazard of impalement:  a) Lower Level at Existing Foundation Wall - Employees were observed walking near several pieces of protruding rebar that were left when the existing foundation wall was partially removed and exposed to an impalement hazard, on or about 10/21/2015.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  a) Job Site - A laborer was observed mixing ESSROC, Type S, Brick-Loc which contained Portland cement and was not provided training in recognizing the hazards associated with the product, on or about 10/21/2015.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the Certification of Corrective Action Worksheet).
