Redfield, Inc. dba Titusville Dairy Queen, 4441 Pennsylvania Route 8 South, Titusville, PA 16354, on or about May 2, 2013 and times prior thereto: A power strip equipped with a flexible cord was used to provide electrical power to a Enerco unvented propane heater. The power strips flexible cord ran from an outlet in the restroom, through a doorway, along the wall surface and to the power strip which accepted the plug for the heater.
