29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):        (a)        Dynamic Design & Manufacturing, Inc. at 6321 Monarch Park Pl., Niwot, CO 80504: On and before 5/3/13, the employer did not protect employees from hazards created by operating ATrump VH-1000 CNC milling machine, in that the interlock system on the door was inoperable. This allows the door to be opened while the machine is running. This condition exposed employees to amputation and struck by hazards.     (b)        Dynamic Design & Manufacturing, Inc. at 6321 Monarch Park Pl., Niwot, CO 80504: On and before 5/3/13, the employer did not protect employees from hazards created by operating a Mori Seiki CNC milling machine, in that the interlock system on the door was inoperable. This allows the door to be opened while the machine is running. This condition exposed employees to amputation and struck by hazards. This condition exposed the employees to the hazard of flying chips and debris.     b)     Dynamic Design & Manufacturing, Inc. at 6321 Monarch Park Pl., Niwot, CO 80504: On and before 5/3/13, the employer did not ensure that the Sterling bench grinder located in the welding area had the work rest to support the work.  This condition exposed the employees to the hazard of flying chips and debris.

29 CFR 1910.215(b)(9):  The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one-fourth inch:           (a)   Dynamic Design & Manufacturing, Inc. at 6321 Monarch Park Pl., Niwot, CO 80504: On and before 5/3/13, the employer did not ensure that the Dayton bench grinders located in the machine shop had the correct distance between the wheel periphery and the end of the peripheral member at the top and did not exceed 1/4". The adjustable tongue was missing from the abrasive wheel grinder. This condition exposed the employees to the hazard of flying chips and debris.       (b)   Dynamic Design & Manufacturing, Inc. at 6321 Monarch Park Pl., Niwot, CO 80504: On and before 5/3/13, the employer did not ensure that the Sterling bench grinders located in the welding area had the correct distance between the wheel periphery and the end of the peripheral member at the top and did not exceed 1/4". Employees operated milling machines without the use of safety glasses. This condition exposed the employees to the hazard of flying chips and debris.      b)               Dynamic Design & Manufacturing, Inc. at 6321 Monarch Park Pl., Niwot, CO 80504: On and before 5/3/13, the employer did not use protective equipment when necessary whenever hazards capable of causing injury and impairment were encountered. Cleaning solution, acetone, hydraulic oil, and used oil was transferred into other containers.  This condition exposed employees to a chemical hazard, skin irritation, and contact dermatitis.
