29 CFR 1910.212(a)(3)(ii): Points of operation of machinery were not guarded to prevent employees from having any part of their body in the danger zones during operating cycles:    (a) Dynamic Design & Manufacturing, Inc., at 6321 Monarch Park Place, Niwot, CO 80504: On or before May 3rd, 2016, the employer did not ensure that the point of operation on an Amada RG 25 Up-Acting press brake was properly guarded to prevent employees from having any part of the their body in the danger zone during operation. Employees placed their hands within one-inch of the point of operation while handling material during press brake operations. This condition exposed employees to amputation hazards.
