29 CFR 1910.242(a): Hand and portable powered tools or equipment were not kept in safe condition: (a) In the Crate Building Area, on or about May 9, 2013 and at times prior to, a spade handle drill was available for employee use with the power cord insulation pulled from the handle exposing the internal wiring, resulting in an electrial hazard.
