29 CFR 1926.102(a)(3): The vision of employees requiring use of corrective lenses in spectacles was not protected by goggles or spectacles of one of the types listed in paragraphs (i) through (iii) of this section. Combined Support Maintenance Shop Phase 1, Guarng Barrigada Complex, Guam: On May 08, 2103, an employee operating a jackhammer breaking concrete was not wearing eye protection designed to be worn over corrective lenses.

29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible: (Construction Reference 1926.103). Combined Support Maintenance Shop Phase 1, Guarng Barrigada Complex, Guam: On May 08, 2103, employees voluntarily wore 3M N95 filtering facepiece respirators while conducting activities such as, but not limited to, scraping, grinding, and chipping. The employer did not provide employees with information in Appendix D.
