29 CFR 1903.19(d)(1): The employer must submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  The employer did not provide documentation that the following cited hazards were abated:  a.  Employees exposed to fall hazards were not provided training on fall protection procedures.  b.  Employees were exposed to fall greater than 6 feet while roofing on a commercial roof andthey were not provided fall protection.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    On or about May 8, 2013 employees who were exposed to fall hazards were not trained in fall protection procedures to recognize and minimize those hazards.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels and 50-feet (15.25 m) wide or less, was not protected from falling by guardrail systems, or safety nets:    On or about May 8, 2013 employees were roofing a commercial roof at a height greater than 6 feet with no fall protection.    Bulldog Construction was previously cited for a violation of this or its equivalent Occupational Safety and Health Standard 29 CFR 1926.501(b)(13), which was contained in OSHA Inspection Number 696198, Citation Number 2, Item Number 1, Issued on 11/6/2012, and was affirmed final order on 2/25/2013, with respect to a workplace located at 2723 North Wynde Passage, Sun Prairie, WI 53590.    Bulldog Construction was previously cited for a violation of this or its equivalent Occupational Safety and Health Standard 29 CFR 1926.501(b)(13), which was contained in OSHA Inspection Number 313133720, Citation Number 1, Item Number 1, Issued on 6/29/2010, and was affirmed final order on 8/6/2010, with respect to a workplace located at 400 Giertson, Stoughton, WI 53589.
