29 CFR 1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required:  (a) The employer did not submit abatement certification and documentation for Citation 1, Item 1 and Citation 1, Item 2 regarding employees not being provided with safety glasses and fall protection, respectively.

29 CFR 1926.102(a)(1): Employees shall be provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents.      On or about April 24, 2015, employees were not provided with safety glasses while using a pneumatic nail gun to install shingles on a residential roof.      Bulldog Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1) which was contained in OSHA inspection number 696198, citation number 1, item number 1 and was affirmed as a final order on February 5, 2013 with respect to a workplace located at 2723 N. Wynde Passage Sun Prairie, WI 53590.

29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.      On or about April 24, 2015, employees installing shingles on a residential roof were not protected from falling to lower levels.  Bulldog Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(10) which was contained in OSHA inspection number 905480, citation number 2, item number 1 and was affirmed as a final order on October 11, 2013 with respect to a workplace located at the corner of Crescendo Drive and Ironwood Drive Sun Prairie, WI 53596.  Bulldog Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13) which was contained in OSHA inspection number 696198, citation number 2, item number 1 and was affirmed as a final order on February 25, 2013 with respect to a workplace located at 2723 North Wynde Passage Sun Prairie, WI 53590.  Bulldog Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501 (b)(13) which was contained in OSHA inspection number 313133720, citation number 1, item number 1 and was affirmed as a final order on August 8, 2010, with respect to a workplace located at 400 Giertson Stoughton, WI 53589.
