29 CFR  1926.20(b)(1): The employer did not initiate and maintain such programs as may be necessary to comply with this part.  a)	Bulldog Construction, LLC. On or about March 4, 2024 at the address of 8540 Hocking Cove. Waterville, OH 43566: The employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment. Such programs would support the employer and employees in identifying and avoiding potential hazards such as falls during job site operations, decreasing the probability of employee injury and/or illness.  Bulldog Construction, LLC was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent 1926.20(b)(1), which was contained in OSHA inspection number 1613546, citation number 2, item number 1a and was affirmed as a final order on September 28, 2022 with respect to a workplace located at 3973 Navarre Blvd. Toledo, OH 43615  Bulldog Construction, LLC was previously cited for a violation of this Occupational Safety and Health Standard 1926.20(b)(1), which was contained in OSHA inspection number 1476628, citation number 1, item number 1a and was affirmed as a final order on July 21, 2020 with respect to a workplace located at 2839 River Rd. Maumee, OH 43537.

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a)	Bulldog Construction, LLC. On or about March 4, 2024 at the address of 8540 Hocking Cove. Waterville, OH 43566: The employer did not ensure that employees had been trained in the appropriate use of fall protection such as but not limited to the use of personal fall arrest systems or alternative measures while performing roofing activities from elevated surfaces greater than six feet from the level below.  Bulldog Construction, LLC was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent 1926.503(a)(1), which was contained in OSHA inspection number 1613546, citation number 2, item number 1b and was affirmed as a final order on September 28, 2022 with respect to a workplace located at 3973 Navarre Blvd. Toledo, OH 43615  Bulldog Construction, LLC was previously cited for a violation of this Occupational Safety and Health Standard 1926.503(a)(1), which was contained in OSHA inspection number 1476628, citation number 1, item number 1b and was affirmed as a final order on July 21, 2020 with respect to a workplace located at 2839 River Rd. On or about March 4, 2024 at the address of 8540 Hocking Cove. Waterville, OH 43566: The employer did not ensure each affected employee used appropriate eye or face protection while utilizing pneumatic nail guns during roofing work activities on residential structure. Employees working in this condition were exposed to potential struck by hazards (flying projectile/ricochet nails).  Bulldog Construction, LLC was previously cited for a violation of this Occupational Safety and Health Standard1926.102(a)(1), which was contained in OSHA inspection number 1613546, citation number 1, item number 2 and was affirmed as a final order on September 28, 2022 with respect to a workplace located at 3973 Navarre Blvd. Toledo, OH 43615

On or about March 4, 2024 at the address of 8540 Hocking Cove. Waterville, OH 43566: The employer did not ensure employees were protected from falls while performing roofing work activities (installing asphalt shingles) atop a single-story residential structure with an 12:12 roof pitch. Employees operating in this conditions were exposed to falls of approximately 14ft or higher to the ground below.
