29 CFR 1910.305(g)(2)(iii): Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws. a. Welding and Fabrication Area - On or about May 15, 2013, an extension cord was being utilized to energize a hand tool - hand grinder - (DeWalt # 28402) whose electrical cord outer sheathing was pulled back from its plug. Employees were thereby exposed to electrical hazards.

29 CFR 1910.334(a)(2): Visual inspection - portable cord and plug-connected equipment and flexible cord sets (extension cords) shall be visually inspected before use on any shift for external defects and for evidence of possible internal damage; a. Fabrication and Welding Area - On or about May 15, 2013, two of three extension cords plugged into a junction electrical box were showing evidence of physical damage. Employees were thereby exposed to electrical hazards. The evident physical damage were the following; 1. One extension cord (from the junction box to the upper part of the building) showed damage on the outer jacket. 2. Other extension cord (from the junction box to the welding table) showed its outer insulation pulled away from its plug.
