29 CFR 1910.179(n)(2)(ii): The load was not attached to the load block hook by means of slings or other approved devices: a) On or about February 5, 2015, at the above located workplace employees were required to ensure that loads were attached to a load block hook by a Earth-Chain, Model ELM-3000, magnet. The magnet used had a modified handle and was not an approved device. Employees were thereby exposed to the hazard of falling loads.
