29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of worksite, materials, and equipment to be made by a competent person (i.e., a person who is capable of identifying existing and predictable hazards in the surroundings or working conditions which are unsanitary, hazardous, or dangerous to employees, and who has the authority to take prompt corrective measures to eliminate them): (a) Carpentry Solutions, Inc., 3420 Irving, Denver, CO: On and before 5/03/13, the employer did not ensure a "competent person" conducted frequent inspections of the job-site to ensure the use of fall protection by employees working from heights above 6 feet. As a result, employees were observed installing soffit panels from an unguarded second story balcony deck. Condition exposed employees to a fall hazard of approximately 10 feet. (b) Carpentry Solutions, Inc., 3420 Irving, Denver, CO: On and before 5/03/13, the employer did not ensure a "competent person" conducted frequent inspections of the job-site to ensure the use of fall protection by employees working from heights above 6 feet. As a result, employees were observed installing soffit panels while standing on the lower roof section without fall protection. Condition exposed employees to a fall hazard of approximately 15 feet. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

This condition exposed employees to a fall hazard of approximately 10 feet. (b) Carpentry Solutions, Inc., 3420 Irving, Denver, CO: On and before May 3, 2013, employees were observed installing soffit panels while standing on the first story roof without fall protection. This condition exposed employees to a fall hazard of approximately 15 feet. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
